If you received a bill from Centurion Anesthesia for services provided at an ambulatory surgery center or local practice, you can make a Credit Card payment by calling our office at 718-550-8600 x110.
To make a payment by mail please remit to: Centurion Anesthesia, 811 Wilson Street, Valley Stream, New York 11581.
We accept most commercial health insurance, self-pay and out of network benefits based on plan provisions. If you provided us with your insurance information, we billed your insurer as well. You may need to contact your insurance company to encourage them to cover all charges they are responsible for. Subsequent to payment from your insurer, you are responsible for any amounts not covered and will be billed accordingly.
